Syria rebels win support from Britain, battle in Damascus

AMMAN (Reuters) - Syrian government troops backed by tanks battled to oust rebel forces from an opposition stronghold in a Damascus suburb on Tuesday in the heaviest fighting in the capital for months.

In the country's north, rebel fighters stormed an air defense base that President Bashar al-Assad's military had used to bombard areas near the Turkish border.

On the international front, the Turkish foreign minister said NATO states had agreed to supply Turkey with a Patriot missile system to defend against Syrian cross-border shelling.

Although any such deployment would be for defensive purposes only, it nonetheless marked a hardening stance in the foreign effort to remove Assad.

The rebels also received a diplomatic lift with Britain officially recognizing the opposition Syrian National Coalition, set up this month to boost their chances of securing foreign aid and arms, as the Syrian people's legitimate representative.

It was the ninth country to do so following France, Turkey and the Gulf Arab states.

After months of slow progress marked by poor organization and supply problems, the rebels have captured several army positions in outlying regions in the last week, including a Special Forces base near Aleppo, Syria's commercial hub.

They are also trying to take the 20-month-old revolt to the heart of Damascus, Assad's seat of power, and have dubbed this week "March to Damascus Week".

On Tuesday, elite Republican Guard troops attacked the rebel stronghold of Daraya on the city's southwestern edge but met resistance from rebel fighters of the Free Syrian Army, opposition sources said.

Seven civilians and three rebels were killed in fighting and bombardments on Daraya, the sources said.

Video footage showed the body of a baby at a hospital. A young couple died from shrapnel when artillery hit the basement of a building in which they were sheltering, activists said.

"The Republican Guards are hitting the town with tanks, artillery and rockets. Most civilians had fled and those who have stayed are trapped with nowhere to escape," Abu Kinan, an activist in the Daraya, said by phone.

A Western diplomat following the fighting said Assad had to show he could repel the rebel challenge to Damascus.

"He has to show that letting the bases fall in and round Damascus is only temporary while he begins to consolidate resources and personnel and deals with the struggles in the east," the diplomat said.

Also on Tuesday, two mortar rounds hit the Information Ministry building in Damascus, causing damage but no casualties, state television said. It blamed "terrorists" for the attack, the usual government term for anti-Assad forces.

In total, 100 people were killed in violence on Tuesday, 64 of them civilians, the pro-opposition Syrian Observatory for Human Rights said.

In the north, opposition sources said rebel fighters had captured sections an air defense base at Sheikh Suleiman, 18 km (11 miles) from the Turkish border and 30 km (20 miles) northwest of Aleppo.

The fighters seized three artillery pieces and large stocks of explosives but would withdraw to avoid retaliatory air strikes, opposition source said.

"Assad's forces use the base to shell many villages and towns in the countryside. It is now neutralized," one said.

MISSILES ON THE BORDER

In Ankara, Turkish Foreign Minister Ahmet Davutoglu said NATO states had agreed to supply Turkey with an advanced Patriot missile system to defend against Syrian attacks.

Talks on its deployment are in the final stage, he said. Only the United States, the Netherlands have the appropriate system available.

In recent months artillery and mortar fire from Syria has landed inside Turkey, increasing concern that the anti-Assad uprising could turn into a regional conflagration.

Turkey has often scrambled fighter jets along the border in a warning to Damascus as Syrian war planes and helicopters bomb rebel positions just a few kilometers (miles) from Turkish soil.

NATO Secretary General Anders Fogh Rasmussen said on Monday that any missile deployment would be a defensive measure and not to enforce a no-fly zone over Syria.

Although the rebels have taken large swathes of land, they are almost defenseless against the government's air force. They have called for an internationally enforced no-fly zone, a measure that helped Libyan rebels overthrow dictator Muammar Gaddafi last year.

Despite strong censure of Assad, Western powers have shied away from direct military involvement.

But the political campaign against Assad took a step forward on Tuesday when British Foreign Secretary William Hague announced that Britain recognized the new opposition coalition as the Syrian people's sole legitimate representative.

The British move goes further than the European Union, which recognized the coalition but not exclusively. Washington has also stopped short of full recognition.

Britain says no option is off the table but Hague told parliament no decision had been taken to supply military aid.

"It's a morale boost. It gives some credibility to the opposition, and it could lay the platform practically for more effective ways of channeling support, plus some quasi-military support," said David Butter, Middle East expert at London-based thinktank Chatham House.

An estimated 38,000 people have been killed in Syria since an Arab Spring-inspired uprising against Assad began in March last year. The initially peaceful protests turned into an armed rebellion after a government crackdown.

ScienceDaily (Nov. 6, 2012) ? A new study shows that defibrillators -- devices designed to detect and correct dangerous heart rhythms -- can help people with heart disease live longer, and with a much better quality of life, than they do now.

A small, very simple change in the way physicians set or programmed the device led to a dramatic 80 to 90 percent reduction in inappropriate therapies -- potentially painful and anxiety-provoking shocks delivered for rhythms that aren't dangerous or life threatening. And, to the authors' surprise, the new programming also significantly increased survival, lowering the risk of death by 55 percent compared to patients whose devices used traditional programming. This reduction is above and beyond the usual decrease in mortality associated with defibrillator therapy, leading to an overall 70 percent reduction in death.

The MADIT-RIT trial, published November 6 in the New England Journal of Medicine and presented at the American Heart Association's Scientific Sessions in Los Angeles, was led by Professor Arthur J. Moss, M.D., an expert in the treatment and prevention of cardiac arrhythmias and sudden cardiac death. Moss' team found that simply raising the heart rate at which the device is set to fire -- deliver therapy -- made all the difference.

"The way we've been using implantable defibrillators for the last 20 years has really been less than optimal," said Moss, longtime professor of Cardiology at the University of Rochester Medical Center. "The extent to which the new programming reduced death and inappropriate therapies was quite striking and has the potential to beneficially affect a wide spectrum of patients who are at risk for sudden death or rhythm disorders."

Implantable cardioverter defibrillators or ICDs constantly monitor the rate and rhythm of the heart and are supposed to deliver electrical shocks in response to very fast and potentially fatal heart rhythms. In 2002, Moss and the MADIT (Multicenter Automatic Defibrillator Implantation Trial) research group showed that ICDs are extremely effective in preventing death in patients at risk of irregular heart rhythms and sudden cardiac death, including individuals who've suffered a heart attack. The work changed medical guidelines nationwide, making thousands of heart attack survivors eligible for ICD therapy. Currently, around 200,000 ICDs are implanted in the U.S. every year.

But, according to a 2008 study in the Journal of the American College of Cardiology, approximately 20 to 25 percent of defibrillator therapy is inappropriate, meaning shocks are delivered in response to less dangerous rhythms that aren't likely to pose any immediate danger to patients. Such shocks are not only painful, but can take an emotional toll as well. Data from a 2002 trial comparing anti-arrhythmic drugs and defibrillators suggested that shocks are associated with reduced mental well-being. And a recent review of more than 45 studies found that such emotional distress is not uncommon, reporting that between 11 and 28 percent of ICD patients had some form of depression and between 11 and 26 percent had an anxiety disorder.

Moss' team conducted the MADIT-RIT (Reduce Inappropriate Therapy) trial, which was sponsored by Boston Scientific, to determine if different ways of setting the device -- a complex process performed by a heart rhythm specialist prior to device implantation in a patient's chest -- could reduce the occurrence of inappropriate therapy. Boston Scientific develops and markets medical devices, including ICDs and cardiac resynchronization therapy defibrillators or CRT-Ds.

From September 2009 through October 2011 the team enrolled 1,500 patients in 98 hospital centers in the United States, Canada, Europe, Israel, and Japan. All patients had heart disease and received a Boston Scientific ICD or CRT-D.

Currently, most defibrillators are set to initiate therapy when the heart rate exceeds around 170 beats per minute, but rates of 180 or 190 are not always dangerous, are usually short-lived, and could be related to increased activity or exercise. Unfortunately, Moss says, defibrillators aren't very good at differentiating benign from malignant rhythms in this "in-between" range.

Setting the device to fire at a higher rate of 200 beats per minute reduced the risk of experiencing a first inappropriate therapy by 79 percent compared to standard programming. Fewer shocks also corresponded with less energy delivered to the heart, which study authors believe contributed to the reduced risk of death.

"There is considerable research to suggest that there is a small amount of damage to the heart muscle with each delivered shock," added Moss. "If we can eliminate the unnecessary shocks, this is going to be associated with less heart damage and improved outcomes."

The MADIT-RIT trial is the first large-scale, randomized study designed to evaluate specific programming features to reduce inappropriate therapy in patients with ICDs. The trial's sponsor, Boston Scientific, was not involved in data collection or data analysis. Moss, who has led the MADIT trials since their inception in the 1990s, holds no stock in any device company, has never been a member of any corporate speakers bureau, and since Dec. 1, 2008, has chosen not to accept honoraria from Boston Scientific for any professional activity.

In addition to Moss, Wojciech Zareba, M.D., Ph.D., Mary Brown, David Huang, M.D., Helmut Klein, M.D., and Scott McNitt from the University of Rochester Medical Center participated in the research. Researchers and physicians from Henry Ford Hospital, Hospital of the Good Samaritan, Duke University Medical Center, Tufts-New England Medical Center, Columbia University, Institute for Clinical and Experimental Medicine, University of Iowa Health Care, Tokyo Women's Medical University, and Loyola University Medical Center also contributed to the research.

Moss, who's spent a large part of his career working to improve ICD therapy, concludes, "When the findings were first uncovered there was a sincere, general enthusiasm that we had finally come to improve the therapy we've been using for 20 years, and that we've made true progress in making ICD therapy safer, more effective, and more acceptable to patients."

In the past, we have solicited a main entertainment sponsor who would cover the costs of all the bands. However, many times that money would only cover one band, and we would have to rely on many ?volunteer? bands to come and play for free. We very much appreciate the generosity of these bands, but would like to be able to at least cover gas and food expenses for the bands this year.

So, what we?re going to try is finding an individual sponsor for each band we would like to employ for the festival. This means that we need your help in securing sponsorships for many of the bands! And, as a reward, any band sponsor will receive VIP backstage passes and have access to all the bands, as well as complementary snacks and drinks backstage. Some of the bands will cost very little, and should be affordable for any small business in the Wiregrass. Il Cantuccio feels like home. The decorations, the kitchen, the staff, the food? ? everything seems more an extension of an Italian relative?s dining room than a restaurant tucked away on a quiet corner in Northern Liberties. The place is small, housing only a handful of wooden tables. Exposed brick, colorful tiles and little Italian trinkets feel like odds and ends picked up over a lifetime.

The food here pairs nicely with these comfy vibes. Advised by Giuseppe, our waiter, and his mother (both long?time I.C. fixtures) we started with the Misto appetizer ($8). Soft pepper jack and provolone cheeses, caramelized carrots, stuffed tomatoes and string beans fill the plate to its brim. Little is touched by the chef: the ingredients? freshness and natural flavors become the main attraction. It?s all delicious, but the cheeses truly melt on the tongue.

Our entr?es followed quickly. Enthusiastically suggested by Giuseppe, the Spinach Ravioli? ($20) was bathed in a pink sauce with peas and mushrooms. The richness of the tomatoes and thinly?sliced mushrooms, along with the sweetness of the peas hit just the right spot alongside the ricotta cheese bubbling inside the pasta. We also tried a pasta special with chicken sausage, cherry tomatoes and broccoli, splashed with a garlic sauce ($20). The pasta itself was unremarkable, and the sauce just barely missed its mark. But the sausage was bursting at its seams, and the vegetables were flavorful. The broccoli especially came in handy, soaking up the broth and complementing its otherwise lackluster flavors perfectly.

By the time for dessert, we were nearly stuffed by the generous portions of the earlier dishes. But we couldn?t pass up the Homemade Tiramisu ($5), the Chocolate Cream Puff ($5) or the Chocolate Cannoli ($5) when they arrived on an artfully?arranged platter. The cannoli emerged as the clear standout, but everything had us finishing the meal on a high note.

From start to finish, a repast in this tiny restaurant satisfies. For those familiar with La Locanda, the Italian BYO of the same owner in Old City, Il Cantuccio doesn?t stray far in quality from its food or atmosphere. If anything, it?s warmer and more welcoming than its counterpart. BYO and parents?weekend friendly, you?d be hard?pressed to find an occasion when Il Cantuccio doesn?t work.

TV: Israel security heads halted Iran attack alert

JERUSALEM (AP) ? Israel's prime minister and defense minister ordered the military to go on alert to prepare to attack Iran's nuclear program two years ago, but backed off following opposition from top security officials, an Israeli news show claims in a report to be aired Monday night.

A pre-broadcast news release from Channel 2's Uvda (Fact) show did not say whether a final decision to attack was made.

However, it says the alert order quickly met opposition from then-military chief Lt. Gen. Gabi Ashkenazi, who warned that Israel's enemies would notice the measure and that in itself might touch off a war.

"This accordion produces music when you play it," the statement quotes him as saying. "This is not something you do if you are not sure you want to end up with a military operation."

The statement also said Meir Dagan, then heading the Mossad spy agency, accused Prime Minister Benjamin Netanyahu and Defense Minister Ehud Barak of acting illegally by not seeking formal approval from the Cabinet of ministers.

Netanyahu and Barak "simply tried to steal a decision to go to war," Uvda quotes him as saying.

Netanyahu spokesman Mark Regev said the government wouldn't comment until the full show airs. An excerpt broadcast Sunday night showed Barak saying Ashkenazi told him the military wasn't able to carry out the attack.

Ashkenazi denies that, saying he instead told Barak that an attack at that time "would be a strategic mistake," the statement said.

The two former security officials could not be reached for comment.

Israel does not believe Tehran's claims that its nuclear program is peaceful and designed to produce energy and medical isotopes. It considers a nuclear-armed Iran to be a threat to its survival because of its nuclear program, arsenal of weapons capable of striking the Jewish state, support for anti-Israel militants groups and frequent calls for Israel's destruction.

Netanyahu has repeatedly said the threat of force must be seriously considered, recently warning that the world has until next summer at the latest to keep Iran from building a bomb.

Union workers strike at Raley's supermarket chain

(AP) ? Union workers at Raley's supermarket chain, which operates more than 100 stores in California and Nevada, went on strike Sunday after last-ditch efforts to reach a new contract failed.

The announcement by union leaders came a few hours after a midnight deadline passed and both sides said the existing labor contract was being extended on an hour-by-hour basis at the request of a federal mediator.

The union said in a statement that "by late on Saturday evening it had become obvious Raley's offer to extend the contract for an additional three days and return to the bargaining table" was a delaying tactic. Picket lines will begin early Sunday, said Ellen Anreder, a spokeswoman for the United Food and Commercial Workers Union.

Ralye's spokesman John Segale said that "it is unfortunate that after 15 months of talks and nearly 60 negotiation sessions, we were not able to agree on a new contract since it is clearly understood that we must reduce our operating costs to become more competitive against non-union retailers."

He said the chain has "plans in place to deal with any job actions to keep our stores open and operating to meet the needs of our customers."

On Saturday, the two sides held a third day of bargaining aimed at heading off the first strike in Raley's 77-year history.

The grocer has 128 stores in Northern California and Nevada.

The two sides have been at odds over a proposed wage freeze, elimination of premium pay for Sunday shifts and health care benefits.

"Raley's has tried to dictate a laundry list of takeaways including devastating members' and retirees' health and welfare plan and nothing has changed. It's clear from their actions it's time to teach them the meaning of respect," the union statement said.

Raley's management had threatened to impose a contract with its terms on Thursday, but when a federal mediator became involved in the talks, the company said it would hold off until midnight Saturday.

Raley's says it needs to cut costs in the face of a weak economy and competition from nonunionized companies that also sell groceries, such as Wal-Mart Stores Inc.

Union officials say the chain has not agreed to a full audit of its finances, failing to make the case for the concessions

West Sacramento-based Raley's is a privately owned company that employs 13,000 people at 115 stores in California and 13 in Nevada operating under the Raley's name, as well as Bel Air Markets, Nob Hill Foods, Food Source and Aisle 1 Fuel Stations, according to its website.

The United Food and Commercial Workers says it represents 7,400 of the chain's employees.
